Application forms

A note about level before you download an application form: The level for these masterclasses is described as advanced/professional. This means that you may be:

• professional, semi-professional or
• have completed at least one year in full-time training for a career in dance, and have reached your 16th birthday by the beginning of the course. You should be of the highest standard at Advanced Level (RAD Advanced 2 or equivalent).

For the WORD document:  If a window appears asking you to open/save or cancel. Press SAVE and then save the file to wherever you want and then open it to edit it. Once you have edited it with all your details save it in your own name and then email it to us at administrator@balletmasterclass.com along with a photograph of yourself in a normal ballet position (1st Arabesque is good).

For the PDF version: You will need to print this off and then fill it out and post it to us at the address at the bottom of the form, or having filled it out then scan it and email it back to us along with a full length photograph.
